Medication Reconciliation Project

Reflecting on this project brings me to think about possible challenges and what is being done correctly.  I feel my client has a good grasp on living a healthy lifestyle, they have a healthy diet, exercise regularly, see their doctor regularly, and adhere to their medications.  My client is older than 65 so it puts them in a vulnerable population to health risks and to sensitivities to medications.  I see that as a potential safety concern and special attention needs to be focused on the need for medications and the impact the medications have on the body.  The client is very intelligent and has a great grasp on why they are taking the medications and exactly what they do to the body.  The medications that may have clashed were an NSAID as they can impede the function of the beta blocker they are on.  A letter from their doctor was provided to me that explained their good health and what their current cardiac status is.  I am still in the process of translating it as it is written in German.

            Overall, I feel that the biggest challenge would be travel and if they had to replace medications while being abroad from Germany.  I feel confident that they have established connections in the US so that in the case of needing to replace mediations they would be able to do so. Their lifestyle and their holistic approach to their wellbeing and adherence to medications is of sound judgement.
